Note: Due to construction, the Venue for this program has been changed from the Doubletree to the Hilton Scottsdale (about 1 mile north).

 

The Forum features engaging presentations, round-table discussion, state nursing update plus networking with nurse leaders from a variety of settings around the state.  If you are a Nurse in (or aspiring to) a leadership role, this program is for you!

Care Coordination:  Inspiring Transformative Change in Complex Healthcare Environments

Optimizing the patient experience while maximizing outcomes with proper stewardship of resources is one of the greatest challenges confronting healthcare today.  Whether out of interest or necessity, organizations around the state are improving performance and transforming care coordination and delivery.  Join us as we explore how nurse leaders from several different settings are championing and inspiring transformative changes plus receive updated bench-marking data for Arizona on readmissions and quality improvement.

   

Presenters

Raianne Melton, RN, BSN, CHPN

Raianne Melton is the Director of Clinical Strategy and Supportive care for Banner Hospice and Palliative Care.  As a practicing RN, Raianne has worked across the life span of patients.  In pursuing her passion for patient care, she has also worked in a variety of settings including hospitals, schools, and homes.  Raianne spent several weeks working in a clinic and remote areas of Uganda providing basic health care to those who typically had no access to medical care.  Transitioning to hospice and palliative care 13 years ago, Raianne has found an avenue to provide excellent patient care for patients at the end of life.  Since moving to Raianne has served as a hospice admission nurse, manager of hospice intake and admissions, director of a hospice inpatient unit, director of education and staff development, clinical director, hospice administrator and director of a home based palliative care program.  She describes her job as a privilege—a privilege to serve patients and families and a privilege to mentor those staff and leaders that will serve us all in the future. Banner Home Based Palliative Care Program received the The Best of Best award for Post Acute Alignment.

 

Amy Nunemaker, RN

Amy Nunemaker has been an ICU RN since 2001.  She has worked in many different types of ICUs ranging from trauma, neuro to medical and cardiovascular.  She has worked in cities such as Austin TX, Seattle WA, Phoenix, Tucson and Flagstaff AZ.  She has been settled in Flagstaff AZ since 2009 starting as a bedside nurse on nights in the ICU and is now a day charge nurse in the ICU.  Over the years she has created a CRRT program for the Critical Care Department that she over sees providing all the education and follow-up.  She has been a member of several different committees such as bereavement program, mentorship program, employee engagement program.  She is a mentor, leader and resource for the nurses she works side by side with.  She has received the NAH Healthcare Charge nurse of the year in 2015.  In the fall of 2018 the Critical Care unit based council wanted to create a program that focused on honoring the families for making the difficult decision of making their loved one an organ donor during such a tragic time.  She created a Hero in the House Honor walk for a pediatric donor patient in November of 2018 which she received a LOV Award and Daisy Award.  Amy decided to take on this project and create a multi faceted program to honor the family and the patient for saving other lives. In her free time from FMC she is starting a second career as a SANE Nurse for Northern AZ. 

 

Keith Parker, MBA
As the Chief Information Officer, Keith Parker is responsible for all information system functions at Health Current, which include core HIE services, as well as population health and data analytics. Mr. Parker Joined Health Current in November 2016 and has over 30 years’ experience in healthcare. His area of expertise is in health information technology implementation and use, process improvement, along with change management methodology and implementation in healthcare. He holds undergraduate degrees in business administration, economics and an MBA from Saint Martin’s University. 

 

 

 

Luke Smith, DNP, RN, NE-BC

Dr. Luke Smith has over 15 years’ experience in healthcare, with expertise in community outreach, public health programs, acute care leadership and quality improvement. He has worked as RN in telemetry, the ED and Interventional Radiology and is currently also an adjunct professor at Grand Canyon University. Mr. Smith is passionate about care coordination and currently he has joined Health Services Advisory group (HSAG), the Arizona Quality Innovation Network - Quality Improvement Organization (AZ QIN-QIO). He serves as the Director of Care Coordination for Arizona and California, communicating, consulting to assist with interventions related to goals, objectives, and measures. Also providing technical assistance and recommendations as needed.
